FORMULATION:
Each vial contains:

• Doripenem Monohydrate equivalent to Doripenem…500mg

For Patient ≥18 years old:

1. with Complicated intra-abdominal infection: 500 mg 1 hour IV infusion every 8 hours for 5-14 days.

2. with Infection of the urinary tract including pyelonephritis:500 mg 1 hour IV infusion every 8 hours for 10 days and may be extended up to 14 days for patients w/ concurrent bacteremia.

Patient with Renal impairment: CrCl ≥30 to ≤50 mL/min 250 mg IV (over 1 hour) every 8 hours, >10 to <30 mL/min 250 mg IV (over 1 hour) every 12 hr.
